What is a 504 Plan and How Can it Help My Teen? Tip Sheet- The 504 plan tip sheet the Transitions ACR created describes solely what exactly the plan is and how you can create the plan to help your teen in school. You may think this plan is like a IEP yet is not. This tip sheet will explain the difference between the two. Just click on the picture link for more information If you’re having trouble in school and they are not meeting your needs you have the right to request an IEP evaluation (Individualized Education Plan). This plan helps accommodate your needs and they work with you more closely than in a regular classroom.
